    SMR : Le service médical rendu par ELIQUIS (apixaban) est important dans la nouvelle indication de l’AMM « Traitement des événements thromboemboliques veineux (ETEV) et prévention de la récidive d’ETEV chez les patients pédiatriques âgés de 28 jours à moins de 18 ans ».

    ASMR : Prenant en compte : • les données d’efficacité d’apixaban en population pédiatrique, issues principalement d‘une étude clinique dont l’interprétation ne peut être que descriptive dans la mesure où aucune hypothèse statistique n’a été formulée, qui suggèrent une efficacité similaire à celle des traitements anticoagulants standards par héparine et/ou AVK dans le traitement des événements thromboemboliques veineux et la prévention de leur récidive, • les données de tolérance issues de cette étude, qui ne permettent pas d’exclure un risque de saignements mineurs plus élevé que sous traitement anticoagulant standard par héparine et/ou AVK, • le niveau de preuve de ces résultats qui reste limité, • le profil de tolérance globalement similaire à celui de la population adulte, • l’absence de données comparatives versus XARELTO (rivaroxaban), 1er AOD mis à disposition en France dans cette indication (comparaison directe non attendue compte tenu de leur développement concomitant), • les incertitudes sur son efficacité, sa tolérance et son utilisation à plus long terme en pratique courante, notamment chez les enfants les plus jeunes peu nombreux dans l’étude pivot, la Commission considère que ELIQUIS (apixaban) n’apporte pas d’amélioration du service médical rendu (ASMR V) dans la prise en charge des événements thromboemboliques veineux dans la population pédiatrique après au moins 5 jours d’une anticoagulation parentérale initiale.

    ASMR : La Commission considère que les spécialités ELIQUIS apportent une amélioration du service médical rendu mineure (ASMR IV) par rapport à la warfarine dans la prévention de l’accident vasculaire cérébral et de l’embolie systémique chez les patients adultes atteints de fibrillation atriale non valvulaire et présentant un ou plusieurs facteurs de risque. Les nouvelles données disponibles ne sont pas susceptibles de modifier l’appréciation précédente de la Commission, qui considère que les spécialités ELIQUIS apportent une amélioration du service médical rendu mineure (ASMR IV) en termes d’efficacité par rapport à l’énoxaparine pour la prévention des événements thromboemboliques veineux chez les patients adultes bénéficiant d’une intervention chirurgicale programmée pour prothèse totale de hanche ou de genou.
